A sample of 1.5 inches from around 200 hair strands (approximating the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for analysis and verification. For EtG, additional tests, and/or screens exceeding 10 panels, a 150mg specimen is advisable. We suggest using a jeweler’s scale for weighing. In the absence of scalp hair, an equivalent quantity of body hair can be obtained. In terms of head hair, we specifically mean scalp hair only. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result revolves around four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample within a lab’s framework. This step ensures that the sample was securely sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any outstanding data entry not offered by an electronic chain of custody system.

Screening consists of a preliminary assessment for substances of abuse. While Screening is a budget-friendly method to exclude drug consumption in the majority of cases, a positive result must be confirmed to hold weight in legal situations. Samples that are potentially positive in Screening necessitate a follow-up confirmation.

Should a sample yield a presumptive positive during Screening, additional hair is retrieved from the initial sample for Extraction. During Extraction, drugs are removed from hair at a substantially lower concentration than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging technique to execute.

Confirmation of any screen-positive result is undertaken via GC/MS, GC/MS/MS, or LC/MS/MS. All samples yielding presumptive positives are cleansed prior to confirmation as required. The complete lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
