Collecting hair is straightforward, tampering is relatively challenging, and it is easy to transport.

A 1.5-inch segment consisting of roughly 200 hair strands (comparable to a #2 pencil's diameter) from the scalp gives 100mg of hair, which is the optimal amount for both screening and confirmation. For EtG, extended panels, or tests beyond 10 panels, a 150mg sample is suggested. It's best to weigh the sample using a jeweler's scale. If scalp hair is not accessible, a similar quantity of body hair may be used. When we mention head hair, we specifically mean scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The process of lab testing for drug detection follows four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial introduction into the lab's system, ensuring the sample is sealed and transported correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any added data entry not covered by an electronic chain of custody.

Screening provides a primary check for drug presence. While this method is an economical way to exclude drug use in most samples, any positive findings must be confirmed for court admissibility. Samples that test positive in Screening need further confirmation.

For samples testing positive during Screening, more hair is extracted from the original sample for the Extraction phase. At this point, drugs are extracted at a lower concentration than alternative methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to conduct.

Positive Screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positive results are cleaned as necessary before confirmation. The complete lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair classification and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
