The laboratory evaluation of a drug test result involves four core stages: Accessioning, Screening, Extraction, and Confirmation.

Samples showing presumptive positivity in Screening require subsequent confirmation.

If a sample is deemed presumptively positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are isolated from hair at significantly diminished concentrations as compared to methodologies like urine or oral fluid, which makes hair drug screening notably challenging.

Positively screened results undergo Confirmation through GC/MS, GC/MS/MS or LC/MS/MS. All samples with suspected positivity are washed as necessary before confirmation. The complete laboratory process, from Accessioning through Confirmation, is conducted under both the CAP (College of American Pathologists) Hair designation and certification to ISO/IEC 17025 standards.
