A 1.5-inch sampling of roughly 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp will produce 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG add-ons and/or tests exceeding ten panels, 150mg of the sample is advised. It's suggested to weigh the sample using a jeweler's scale. If scalp hair isn't available, an equivalent quantity of body hair may be used instead. When mentioning head hair, we only refer to scalp hair. Body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Summary

The primary steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample within the laboratory system. This step includes ensuring the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not done by an electronic chain of custody system.

Screening provides an initial rapid examination for drugs. While this step is efficient for excluding drug usage for most samples, a positive result in Screening must be authenticated to be valid in legal situations. Samples that are tentatively positive in Screening require further confirmation.

Should a sample be tentatively positive during Screening, additional hair is taken from the original specimen for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging approach to perform.

Confirmation for any positive Screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with tentative positive results are cleansed before confirmation if necessary. The complete lab process from Accessioning to Confirmation operates under the CAP (College of American Pathologists) Hair designation and follows the ISO / IEC 17025 standards.
