A sample measuring 1.5 inches consisting of around 200 hair strands (akin to the diameter of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is ideal. We recommend using a jeweler’s scale for weighing the sample. If scalp hair is not available, an equivalent volume of body hair may be used. When mentioning head hair, it exclusively refers to scalp hair. Body hair encompasses all other hair forms (e.g., facial, axillary, etc.).

Process Overview

There are four primary stages in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a specimen into the laboratory's system. This entails checking that the sample was adequately sealed and dispatched, issuing a random LAN (Laboratory Accessioning Number), and performing any leftover data entry absent in an electronic chain of custody system.

Screening includes an initial swift analysis for drug abuse. Though Screening is a budget-friendly approach to exclude drug consumption in most specimens, a positive screening result requires confirmation to stand in a court of law. Any samples that show a presumptive positive in Screening need secondary verification.

If a sample shows a presumptive positive in the Screening phase, more hair is extracted from the initial sample and prepared for Extraction. This phase extracts drugs from hair in much lower concentrations compared to other methods (such as urine or oral fluid), highlighting why hair drug screening is the most challenging method to execute.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The full laboratory progression, from Accessioning through Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and is aligned with ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
