What it is, How it works

When sampled from near the scalp, hair can reveal alcohol and drug use for up to about three months. It's easy to gather, fairly resistant to tampering, and simple to transport.

A close-to-scalp 1.5-inch segment of roughly 200 hair strands (equivalent to a #2 pencil) yields 100mg, ideal for testing and confirmation. For EtG, supplemental tests, or tests exceeding 10 panels, a 150mg specimen is advised. Using a jeweler's scale for weighing the sample is suggested. If scalp hair is not available, an equivalent amount of body hair can be used. References to head hair pertain only to scalp hair, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results encompasses four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of processing a sample into a lab’s system. This step ensures the sample was sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry absent from an electronic chain of custody system.

Screening conducts a preliminary rapid assessment for drugs of abuse. Although Screening is an economical approach to exclude drug use in most samples, a positive screen mandates confirmation for court admissibility. Samples deemed presumptively positive during Screening require further confirmation.

If the Screening phase finds a sample presumptively positive, additional hair is extracted from the original specimen for the Extraction process. Here, drugs are isolated from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), which makes hair drug analysis the most challenging to perform.

Confirmation of any positive screen is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presupposed positive samples are adequately washed. Every step from Accessioning to Confirmation is reviewed following CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
