What it is, How it works

Hair captures a prolonged record of alcohol and drug use by locking biomarkers within the fibers of the developing hair strand. When cut near the scalp, hair offers up to a roughly 3-month detection period for alcohol and drugs. Hair is straightforward to gather, relatively challenging to tamper with, and convenient to transport.

A 1.5-inch lock of approximately 200 hair strands (roughly the gauge of a #2 pencil) nearest the scalp yields 100mg of hair, the optimal specimen for testing and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advised. We advise using a jeweler's scale to weigh the sample. If head hair is unavailable, the same amount of body hair may be used. When mentioning head hair, we mean scalp hair strictly. Body hair covers all other hair categories (facial, underarm, etc.).

Process Overview

The principal stages involved in processing a drug test in a laboratory include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial processing of a sample into a lab’s system. This entails ensuring the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody process.

Screening involves a preliminary fast check for drugs of abuse. Although Screening is a cost-efficient approach to rule out drug use in most samples, a positive screen must be confirmed to be legally admissible. Any samples that indicate presumptive positivity in Screening necessitate subsequent confirmation.

If a sample shows presumptive positivity at the Screening phase, additional hair is extracted from the initial sample and prepped for Extraction. In this stage, drugs are extracted from hair at a more diluted concentration than with other methods (e.g., urine or saliva), which accounts for why hair drug screening is the most challenging methodology to undertake.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicating presumptive positivity undergo washing before confirmation as required. The complete lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the authorization according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
