What it is, How it works

Hair testing is now a recognized method for identifying drug and alcohol consumption. Through trapping biomarkers in the strands as the hair grows, it provides an extensive history of substance use. Hair collected close to the scalp can reveal drug and alcohol use for up to around 3 months. Collecting hair is straightforward, less prone to tampering, and simple to transport.

A 1.5-inch sample containing about 200 hair strands (roughly the size of a #2 pencil) taken nearest the scalp will yield 100mg of hair, which is the optimal amount for testing. For extensive tests like EtG, add-ons, and/or those that exceed 10 panels, 150mg is advisable. Using a jeweler’s scale to weigh the specimen is suggested. If scalp hair cannot be obtained, an equal amount of body hair might be used. The term head hair signifies scalp hair only, whereas body hair denotes all other types, including facial and axillary hair.

Process Overview

The laboratory processing of drug tests encompasses four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step of incorporating a sample into the lab’s system. This process confirms that the sample was appropriately sealed and sent, allocates a unique LAN (Laboratory Accessioning Number), and includes any necessary additional data entry not provided via the electronic chain of custody.

Screening executes an initial swift evaluation for illicit substances. This method is cost-effective for eliminating drug use possibilities in most samples, yet a positive finding must be confirmed for legal legitimacy. Samples that yield a presumptive positive during Screening must undergo a secondary confirmation.

If Screening indicates a presumptive positive, more hair is taken from the original sample and readied for Extraction. Here, drugs are extracted at significantly lower concentrations than with other methods (*e.g.* urine or oral fluid), which makes hair drug testing considerably challenging to execute.

A positive Screening result undergoes Confirmation by GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are adequately cleansed. The whole laboratory operation, from Accessioning through to Confirmation, is audited under the CAP (College of American Pathologists) Hair standards and accredited to ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
