When acquired near the scalp, hair can offer a detection range of roughly up to three months for alcohol and drugs. Collecting hair is straightforward, moderately resistant to tampering, and easy to transport.

A sample of 1.5 inches, encompassing approximately 200 hair strands (akin to the thickness of a #2 pencil) nearest to the scalp, yields 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advisable. It is suggested to weigh the sample using a jeweler’s scale. If scalp hair is unavailable, an equivalent quantity of body hair can be taken. The term "head hair" pertains solely to scalp hair. "Body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test outcomes includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample undergoes initial processing into a laboratory’s system. This step verifies the proper sealing and shipment of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry absent from an electronic chain of custody system.

Screening offers an initial rapid test for drug presence. While Screening economically rules out drug use in most samples, a positive screen must be authenticated for court admittance. Samples that appear positively in Screening necessitate a further confirmation.

If a sample appears positive at the Screening stage, additional hair is taken from the original sample and prepared for Extraction. Here, drugs are extracted from hair in much lesser concentration than through other methods (e.g., urine or oral fluid), establishing hair drug testing as a challenging methodology to execute.

Confirmation of a positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed as necessary before confirmation. The complete laboratory procedure, from Accessioning to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
