What it is, How it works

When sampled near the scalp, hair can reveal up to roughly a 3-month detection period for drugs and alcohol. It is easy to gather, relatively tamper-resistant, and simple to transport.

A 1.5-inch portion of around 200 hair strands (roughly the thickness of a #2 pencil) next to the scalp provides 100mg of hair, which is the optimal quantity for testing and verification. For EtG, additional testing, and/or tests beyond 10 panels, 150mg of the sample is advised. We suggest weighing the specimen with a jeweler's scale. If scalp hair is not available, a corresponding amount of body hair can be used. By head hair, we mean scalp hair specifically. Body hair includes all other hair types (e.g., facial, axillary).

Process Overview

The four key stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system. This process checks that the sample was properly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data entry not covered by an electronic chain of custody system.

Screening offers a rapid initial assessment for drug misuse. It is a cost-efficient approach to exclude drug usage in most samples, but a positive screening must be validated to be legally acceptable. Samples that presumptively test positive in Screening require further confirmation.

Should a sample test presumptively positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than other methods (e.g., urine or oral fluid), which contributes to the complexity of hair drug screening.

Any positive screening outcome undergoes Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo a wash before confirmation if necessary. The complete laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
