What it is, How it works

Hair testing has emerged as an effective resource for identifying drug and alcohol consumption. Hair retains a prolonged history of alcohol and drug use by capturing biomarkers within the fibers of the expanding hair strand. When gathered near the scalp, hair can offer up to about a 3-month detection timeframe for alcohol and other drugs. Hair is easy to obtain, fairly challenging to tamper with, and straightforward to transport.

A 1.5-inch segment comprising roughly 200 hairs (approximately the diameter of a #2 pencil) closest to the scalp yields 100mg of hair, the optimal amount for both screening and confirmation. For EtG, additional tests, and/or assessments beyond 10 panels, 150mg of the sample is suggested. We advocate for weighing the sample using a jeweler’s scale. If head hair is not available, a matching quantity of body hair can be gathered. When discussing head hair, we mean scalp hair exclusively. Body hair involves all other hair types (facial, axillary, etc.).

Process Overview

The four principal stages in the laboratory handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample into a laboratory’s system. This step verifies that the sample was sealed and dispatched correctly, assigns a random LAN (Laboratory Accessioning Number), and finalizes any extra data entry absent in an electronic chain of custody system.

Screening encompasses an initial expedient check for drugs of abuse. While Screening is an affordable method to rule out drug usage in most samples, a positive screen must be confirmed to be permissible in court. Samples that are tentatively positive in Screening necessitate a secondary confirmation.

If a sample is tentatively positive in the Screening phase, more hair is extracted from the initial sample and readied for Extraction. In this stage, drugs are drawn from hair at a much lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most complex methodology to execute.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
