What it is, How it works

Hair analysis has emerged as an effective method for identifying drug and alcohol consumption. Hair retains a long-term record of alcohol and other substances by encasing biomarkers within its growing fibers. Collected near the scalp, hair offers roughly a 3-month period for detecting alcohol and drugs. It is straightforward to gather, relatively hard to tamper with, and simple to transport.

A 1.5-inch section of around 200 hair strands (comparable to a #2 pencil's thickness) closest to the scalp yields 100mg of hair, the optimal amount for testing and validation. For EtG, supplementary, and/or tests beyond 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is missing, an equivalent amount of body hair can be used. By head hair, we mean only scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test outcome involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab's system. This step involves checking that the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not supplied by an electronic chain of custody system.

Screening is a preliminary quick test for substance abuse. While it is cost-effective for ruling out drug use in most samples, a positive Screening result must be confirmed for court acceptance. Samples that test presumptively positive during Screening need further Confirmation.

If a sample shows presumptive positivity in Screening, more hair is taken from the original sample for Extraction. Here, drugs are extracted from hair at far lower concentrations than with other methods (e.g., urine or oral fluid), making hair drug analysis the most challenging to execute.

Confirmation for any positive Screening output is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Before Confirmation, all presumptive positive samples are cleansed as required. The complete laboratory workflow from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
