When collected near the scalp, hair can offer a detection window of up to 3 months for substances. The collection process is straightforward, resistant to tampering, and easily shipped.

A sample measuring 1.5 inches, comprising roughly 200 hair strands (similar to the width of a #2 pencil) from the scalp provides 100mg, which is ideal for screening and confirmation. For EtG, additional tests, or tests exceeding 10 panels, a specimen of 150mg is advised. It is suggested to weigh the specimen using a jeweler’s scale. If scalp hair isn't available, an equivalent body hair sample can be used. When referencing head hair, it pertains solely to scalp hair. Body hair includes all other types like facial and axillary hair.

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step, involving the integration of a sample into a lab’s system. This verifies the integrity of the sample sealing and shipping, assigns a unique LAN (Laboratory Accessioning Number), and finalizes any data entry not handled by an electronic chain of custody.

Screening is an initial examination for substance abuse. Although it's a cost-effective approach for ruling out drug use in most specimens, a positive result must be confirmed for court admissibility. Samples with presumptive positives in Screening demand further confirmation.

For samples showing potential positives in the Screening, additional hair is taken from the original specimen and prepared for Extraction. Positive Screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are washed before confirmation if needed. The complete process from Accessioning to Confirmation is evaluated under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
