Process Overview

Four central procedures govern the laboratory analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Screening is an initial, swift evaluation for the presence of drugs. While it's economical for excluding drug use in most samples, any positive screen must be confirmed to hold legal weight. Presumptive positives from Screening mandate second-level confirmation.

When Screening indicates a presumptive positive, additional hair is obtained from the original specimen to prepare for Extraction. In this process, substances are extracted from hair at markedly lower concentrations than other techniques (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of any positive screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS methods. Prior to confirmation, all presumptive positive specimens are cleansed as necessary. Throughout, the lab process—spanning Accessioning to Confirmation—is scrutinized under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
