A 1.5-inch section of about 200 hair strands (roughly the diameter of a #2 pencil) nearest to the scalp will yield 100mg, the optimal sample for both screening and confirmation. For EtG, additional tests, and/or exams exceeding 10 panels, 150mg of hair is advised. We suggest using a jeweler's scale for weighing the sample. If no scalp hair is available, an equivalent volume of body hair can be acquired. "Head hair" refers exclusively to scalp hair. "Body hair" covers all other types (e.g., facial, axillary).

Process Overview

There are four critical stages in the lab processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails processing a sample initially into a laboratory's system. This step includes ensuring the sample was appropriately sealed and dispatched, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any leftover information entry not managed by an electronic chain of custody system.

Screening provides a preliminary assessment for drugs. While Screening is an economical option to exclude drug presence in most samples, a positive result necessitates Confirmatory testing for court admissibility. Samples that presumptively test positive in Screening require subsequent confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. During this phase, drugs are drawn from hair at a lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening particularly challenging.

Final confirmation of any positive screening outcome is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The full lab procedure from Accessioning to Confirmation is conducted under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.
