Gathering a sample of 1.5 inches, involving roughly 200 hairs (equivalent in size to a #2 pencil) from the closest point to the scalp yields 100mg, which is the optimal amount for screening and confirmation processes. For tests involving EtG, additional metrics, and/or more than 10 panels, a specimen of 150mg is suggested. We advocate measuring the sample using a jeweler’s scale. If scalp hair cannot be collected, a corresponding quantity of body hair can be used. When referencing head hair, it specifically means scalp hair. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test comprises Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, a sample is first integrated into the laboratory’s system, ensuring it was sealed and transported correctly, assigning it a unique LAN (Laboratory Accessioning Number), and completing any further data requirements not covered by an electronic system.

Screening offers an initial assessment for drugs of abuse. Though Screening is a cost-effective means to exclude drug use in most samples, a positive outcome must be confirmed to be legally valid. Any samples found to be presumptively positive during Screening must undergo further confirmation.

Presumptively positive samples in the Screening phase require additional hair for Extraction, extracting drugs from hair at much lower concentrations than other methods (such as urine or oral fluid), making hair drug screening one of the most challenging methodologies.

Confirmation of any positive result is facilitated using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples presumed positive are cleaned before confirmation as needed. The entire process, from Accessioning to Confirmation, is conducted under both the CAP’s (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
