For tests like EtG, add-ons, or screenings above 10 panels, a 150mg specimen is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is absent, a similar quantity of body hair may be used. In references to head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial intake of a sample into a lab’s system, ensuring that the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not covered by an electronic chain of custody system.

Screening consists of a preliminary and rapid assessment for drug use. Although Screening is a resourceful method for negating drug presence in most samples, a positive screen must undergo confirmation to be valid in legal proceedings. Samples with presumptive positive results in Screening necessitate secondary confirmation.

Should there be a presumptive positive in the Screening phase, additional hair is extracted from the initial sample to prepare for Extraction. This stage involves the extraction of drugs from hair at concentrations significantly lower than those in other methods (e.g., urine or oral fluids), showcasing why hair drug testing is the most intricate methodology to conduct.

Confirmation of a positive screening outcome is performed through techniques like GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed prior to confirmation as required. The entire lab process from Accessioning to Confirmation aligns with both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
