What it is, How it works

Hair examinations have gained recognition as an effective means for detecting drug and alcohol consumption. Hair captures a prolonged history of alcohol and other substances by embedding biomarkers within the strands of growing hair. When gathered near the scalp, hair can offer up to a three-month detection period for alcohol and drugs. Hair is easy to collect, relatively hard to tamper with, and straightforward to transport.

A 1.5-inch segment of approximately 200 hair strands (akin to the diameter of a #2 pencil) closest to the scalp provides 100mg of hair, which is the perfect amount for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest weighing the sample on a jeweler's scale. If scalp hair can't be obtained, a comparable amount of body hair may be used. When mentioning head hair, we refer solely to scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The four key phases in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial sample processing within a laboratory's system. This entails confirming that the sample was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and performing any extra data entry not available through an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs. While Screening is a budget-friendly method to exclude drug use in most samples, a positive screen requires confirmation to be valid in court. Any samples that test preliminarily positive in Screening need a secondary verification.

If a sample tests preliminarily positive during Screening, additional hair is taken from the primary sample for Extraction. In this phase, drugs are drawn from hair at concentrations much lower than other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging technique to execute.

Verification of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The complete lab process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair accreditation and in accordance with ISO / IEC 17025 standards.

Note: Though often termed as "hair follicle tests", the examination itself assesses the hair strand rather than the hair follicle under the scalp.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
