A 1.5-inch section consisting of around 200 hair strands (similar in size to a #2 pencil) nearest to the scalp will yield 100mg of hair, the preferred amount for analysis and confirmation. For EtG, additional tests, and/or assays beyond 10 panels, 150mg of the sample is suggested. It is advised to weigh the hair on a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair is acceptable. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The drug testing laboratory procedure involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the primary step in processing a sample into a lab's system. This process covers ensuring the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry absent from an electronic chain of custody system.

Screening is a preliminary check to detect drugs of abuse. While Screening efficiently excludes drug use in most samples, a positive screen must be verified to be valid in legal settings. Samples that test presumptively positive in Screening necessitate further confirmation.

Should a sample test presumptively positive in the Screening phase, additional hair is extracted from the original sample and set up for Extraction. During this phase, drugs are separated from hair at a lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening presents the most challenging methodology.

Any positive screening outcome is verified using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as required. The complete laboratory process from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
