What it is, How it works

Hair testing has gained recognition as an effective method for identifying drug and alcohol use. By trapping biomarkers in the growing hair fibers, hair offers a long-term record of alcohol and other drug intake. When hair is collected near the scalp, it can offer a detection window of up to around 3 months for alcohol and other substances. Hair collection is straightforward, somewhat challenging to tamper with, and easy to transport.

A sample measuring 1.5 inches, consisting of approximately 200 hair strands (similar to the thickness of a #2 pencil) closest to the scalp, yields 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or analyses beyond 10 panels, a specimen of 150mg is advisable. Using a jeweler’s scale for weighing the specimen is suggested. If scalp hair is not available, an equivalent quantity of body hair can be obtained. When discussing head hair, we specifically mean scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into a laboratory's system. This step includes checking that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and carrying out any additional data entry not covered by an electronic chain of custody system.

Screening refers to an initial rapid assessment for substances of abuse. While Screening is a cost-effective method to exclude drug use in most samples, a positive Screening result requires Confirmation to be acceptable in court. Any samples deemed presumptively positive in Screening need secondary confirmation.

If Screening suggests a positive result, additional hair is extracted from the original sample for preparation in the Extraction stage. Here, drugs are extracted from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting that hair drug screening is the most challenging methodology to perform.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before Confirmation as necessary. The laboratory process, from Accessioning to Confirmation, complies with both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
