The key advantage of these tests is the extended detection time, lasting up to 90 days, conducted under witnessed conditions, making them nearly foolproof. Nevertheless, urine tests remain the most precise for detecting drug use within the last five days.

What it is, How it works

Hair analysis has emerged as a potent means for detecting the consumption of drugs and alcohol. Hair offers a prolonged record of alcohol and drug presence by capturing biomarkers within the fibers of the continuously growing hair. When gathered near the scalp, hair may offer a detection period of up to around three months for alcohol and other substances. Hair collection is straightforward, relatively challenging to tamper with, and simple to transport.

A hair sample measuring 1.5 inches and consisting of about 200 strands (comparable to the thickness of a #2 pencil) closest to the scalp will yield a 100mg hair sample, which is optimal for screening and validation. For EtG, additional tests, or tests exceeding 10 panels, a specimen of 150mg is advised. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair cannot be obtained, a similar amount of body hair can be used. When mentioning head hair, it implies scalp hair only. Body hair pertains to all other hair types (such as facial, axillary, etc.).

Process Overview

There are four primary steps involved in processing a drug test result in a lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initiating the processing of a sample within a lab’s system. This step includes ensuring the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and entering any other data not captured by an electronic chain of custody system.

Screening is a preliminary, swift check for substances of abuse. While screening provides a cost-efficient method to eliminate drug use in most samples, a screen that returns positive must be confirmed to be considered valid for court proceedings. Any samples that give a presumptive positive during Screening require a further Confirmation.

Should a sample show a presumptive positive in the Screening phase, more hair is drawn from the initial sample to prepare for Extraction. This stage involves obtaining drugs from hair at a significantly lower concentration than other methods (e.g., urine or oral fluid), making hair drug analysis one of the most challenging methodologies.

Confirmation of any positive Screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before Confirmation as necessary. The entire laboratory chain from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
