What it is, How it works

Hair analysis has emerged as an influential method for identifying drug and alcohol consumption. Hair captures a long-term record of substances by holding biomarkers within the fibers of the growing hair shaft. When taken near the scalp, hair can offer a detection period extending up to roughly 3 months for alcohol and drugs. Hair is straightforward to collect, relatively hard to tamper with, and uncomplicated to transport.

A 1.5-inch portion consisting of approximately 200 strands (around the size of a #2 pencil) nearest to the scalp will produce 100mg, which is the optimal sample for examination and verification. For EtG, additional tests, and/or assessments beyond 10 panels, a specimen of 150mg is advised. We suggest measuring the sample using a jeweler’s scale. If scalp hair is not an option, an equivalent quantity of body hair can be gathered. In terms of head hair, we are specifically referring to scalp hair. Body hair refers to other types of hair (facial, axillary, etc.).

Process Overview

The key steps involved in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the starter processing of a sample into a laboratory’s system. This step includes ensuring that the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not given by an electronic chain of custody system.

Screening entails an initial rapid check for substances of abuse. While Screening is a cost-efficient method to discount drug consumption in the majority of samples, a positive screen must be confirmed to be permissible in court. Samples that test presumptively positive in Screening require a secondary confirmation.

If a sample shows a presumptive positive in the Screening phase, more hair is taken from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at a concentration much lower than in other techniques (e.g., urine or oral fluid), which explains why hair drug analysis is the most challenging method to execute.

Verification of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as required. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
