What it is, How it works

Hair analysis is now acknowledged as an effective means for identifying the consumption of drugs and alcohol. Hair can document prolonged usage patterns by embedding biomarkers within the strands as they grow. When clipped close to the head, hair offers nearly a three-month timeframe for detecting drugs and alcohol. The collection is straightforward, reducing the chance of tampering, and shipping is simple.

An ideal set for screening and verification is a 1.5-inch piece encompassing roughly 200 hairs (approximate diameter of a #2 pencil), supplying 100mg of hair. For EtG, extended panels, or tests over 10 panels, a total of 150mg is suggested. We advocate using a jeweler’s scale for the specimen's weight. If no head hair is available, a comparable amount of body hair can be used. The term head hair pertains solely to scalp hair, whereas body hair includes various types (facial, axillary, etc.).

Process Overview

The primary processes in laboratory evaluation of a drug test are divided into Accessioning, Screening, Extraction, and Confirmation stages.

Accessioning is the preliminary handling of a sample entering a lab's system, ensuring the sample’s seal and shipment were intact, assigning a unique LAN (Laboratory Accessioning Number), and completing any missing data entry from an electronic chain of custody system.

Screening provides a preliminary analysis for substances of abuse. Although Screening is cost-efficient for initially excluding drug usage in most cases, a positive outcome must be confirmed for courtroom validity. Presumed positive samples in Screening require subsequent affirmation.

When a sample indicates a presumptive positive during Screening, additional hair is extracted from the initial collection to undergo Extraction. At this stage, the drug is isolated from the hair at a concentration much lower than other methods (like urine or saliva), highlighting why hair drug testing is technically challenging.

Confirmation of any preliminary positive result is performed with GC/MS, GC/MS/MS, or LC/MS/MS. Needed samples that are presumed positive undergo washing before confirmation. The entire lab process from Accessioning to Confirmation undergoes scrutiny under the CAP (College of American Pathologists) Hair standardization and the ISO/IEC 17025 accreditations.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
