What it is, How it works

Hair analysis has emerged as an effective method for identifying drug and alcohol consumption. Hair offers an extended record of alcohol and other substances by encapsulating biomarkers within the fibers of the developing hair shaft. When gathered near the scalp, hair can indicate substance use over a time span of about three months for alcohol and other substances. It’s easy to gather hair samples, challenging to tamper with, and straightforward to transport.

A 1.5-inch section of around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal quantity for testing and verifying. For EtG, additional tests, and/or analyses that exceed 10 panels, a sample of 150mg is advised. It’s recommended to measure the specimen using a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair can be used. In reference to head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four core stages in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the foundational handling of a sample into a lab’s system. This incorporates confirming the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not supplied by an electronic chain of custody system.

Screening encompasses an initial rapid assessment for substances of abuse. Though Screening is a financially prudent way to negate drug consumption in most samples, a positive screen must be validated to be permissible in legal settings. Any samples that are tentatively positive in Screening necessitate additional confirmation.

Should a sample be tentatively positive during the Screening phase, more hair is extracted from the original sample and readied for Extraction. Here, drugs are extracted at a concentration much lower than in other methods (e.g., urine or oral fluid), making hair drug analysis the most intricate method to execute.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All initially positive samples are cleansed before confirmation as required. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Note: Though often labeled as "hair follicle tests", the procedure actually examines the hair strand and not the hair follicle beneath the scalp.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
