What it is, How it works

If collected near the scalp, hair offers a detection period of up to about three months for alcohol and drugs. Hair collection is straightforward, relatively resistant to tampering, and easy to transport.

A sample measuring 1.5 inches consisting of roughly 200 hair strands (similar in size to a #2 pencil) nearest to the scalp provides 100mg of hair, which is the ideal quantity for screening and confirming results. For EtG, additional tests, and/or those exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale for specimen measurement is recommended. If scalp hair is unavailable, an equivalent quantity of body hair can be used. When discussing head hair, we refer solely to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The processing of a drug test result in the laboratory involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the preliminary processing of a sample within the laboratory’s system. This step involves ensuring the sample is properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing data entry not covered by an electronic chain of custody system.

Screening represents an initial rapid test for substances of abuse. Though Screening is economical for ruling out drug use in most samples, a positive result necessitates confirmation to hold up in court. Samples presumptively positive in Screening must undergo a secondary confirmation.

If a sample shows positive during the Screening phase, additional hair is taken from the initial specimen for Extraction. In this phase, drugs are extracted from the hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), hence hair drug testing is considered the most challenging method to execute.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed as required before confirmation. The laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation standards of ISO / IEC 17025.
