What it is, How it works

When harvested near the scalp, hair can reveal up to a three-month detection period for alcohol and other drugs. Hair collection is straightforward, hard to tamper with, and simple to transport.

A 1.5-inch portion comprising around 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample for analysis and verification. For EtG, supplementary tests, and/or assessments exceeding 10 panels, a 150mg sample is advised. Using a jeweler’s scale to weigh the specimen is recommended. If scalp hair is inaccessible, a similar quantity of body hair can be gathered. By head hair, we specifically mean scalp hair. Body hair refers to every other hair type (facial, axillary, etc.).

Process Overview

The drug test result processing in a laboratory consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial incorporation of a sample into a laboratory system. This process involves verifying that the sample was correctly sealed and transported, assigning it a random LAN (Laboratory Accessioning Number), and completing any required data entry not provided by an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. Screening is a budget-friendly means to rule out drug usage in most samples, but a positive screen must be verified for court acceptability. Samples that show a presumptive positive in Screening require further confirmation.

If a specimen is presumptively positive at the Screening stage, additional hair is extracted from the original sample and prepared for Extraction. At this juncture, drugs are isolated from hair at significantly lower concentrations than in alternate methodologies (like urine or oral fluid), rendering hair drug screening the most challenging method to execute.

Confirmation of any positive screen result is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned before confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
