Collected near the scalp, hair provides a detection window of up to 3 months for substances. It is easy to collect, challenging to tamper with, and simple to transport.

A 1.5-inch section of roughly 200 hair strands (similar in size to a #2 pencil) closest to the scalp provides 100mg of hair, the optimal quantity for assessment and verification. For EtG, additional screenings, and tests surpassing 10 panels, 150mg of the hair specimen is advisable. Using a jeweler's scale to weigh the sample is suggested. If scalp hair is not available, a comparable amount of body hair is acceptable. "Head hair" specifically refers to scalp hair. "Body hair" encompasses all other hair types (facial, underarm, etc.).

Process Overview

The essential phases in laboratory processing for drug test results include Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample is initially processed into the laboratory's system. This step verifies that the sample was appropriately sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any additional information entry not available through an electronic chain of custody system.

Screening provides a preliminary assessment for substance abuse. Although Screening offers a cost-efficient means to exclude drug use in most samples, a positive result requires confirmation to be valid in a legal setting. Any samples suspected positive during Screening necessitate a second confirmation.

Should a sample test positive during the Screening phase, additional hair is extracted from the initial specimen for preparation in Extraction. Here, substances are retrieved from hair at much lower levels compared to other techniques (e.g., urine or oral fluids), which makes hair drug screening the most challenging technique to execute.

Any positive Screening outcomes undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples are cleansed before Confirmation as needed. The entire laboratory process, from Accessioning to Confirmation, is audited under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
