Hair analysis is regarded as a highly effective method for detecting the usage of drugs and alcohol. Hair strands capture and hold biomarkers within their fibers, offering a historical record of substance use. When collected near the scalp, hair samples can offer an approximate detection timeframe of up to three months for alcohol and drugs. The process of collecting hair is straightforward, with minimal chances for tampering, and easy to transport.

A hair sample of about 1.5 inches in length, consisting of nearly 200 strands (equivalent to the circumference of a #2 pencil) closest to the scalp, provides approximately 100mg of hair, which is optimal for both screening and verification. For EtG, additional compounds, and/or tests exceeding 10 panels, it is advisable to use 150mg of the sample. Using a jeweler’s scale for weighing is recommended. If scalp hair is not available, an equivalent quantity of body hair may be utilized. "Head hair" specifically refers to scalp hair. "Body hair" encompasses all other types, such as facial or axillary hair.

Process Overview

There are four critical procedures in the laboratory analysis of drug testing results: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of integrating a sample into a lab's system for the first time. This step includes ensuring that the sample has been properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any missing data entry not already included in an electronic chain of custody system.

Screening is an initial rapid assessment for the presence of drugs of abuse. Although Screening is an economical means to eliminate drug use in the majority of samples, any positive result must be confirmed to be legally admissible. Samples that test positive during Screening require further confirmation.

Should a sample test positive at the Screening phase, additional hair is extracted from the original sample and readied for Extraction. During this stage, drugs are isolated from the hair in much smaller concentrations compared to other testing methods (such as urine or oral fluids), which is why hair drug testing is considered the most complex method to execute.

To confirm any positive results, GC/MS, GC/MS/MS, or LC/MS/MS techniques are employed. Any samples yielding presumptive positive results are cleansed before confirmation, as necessary. The entire process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair recognition and ISO/IEC 17025 certification requirements.
