A 1.5-inch snippet of roughly 200 hair strands (resembling a #2 pencil's diameter) nearest to the scalp will yield 100mg of hair, the optimal quantity for evaluations and confirmations. For EtG, enhancements, and/or analyses exceeding 10 panels, 150mg of the sample is advised. It's suggested to measure the sample using a jeweler's scale. If scalp hair is not available, a comparable amount of body hair can be utilized. References to head hair specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The process includes four crucial stages in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab's system. This process confirms the sample's proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any supplemental data entry not covered by an electronic chain of custody mechanism.

Screening provides an initial rapid inspection for substance abuse. While Screening is a cost-efficient approach to exclude drug use in most samples, a positive screen mandates confirmation to be admissible in legal proceedings. Any samples preliminarily positive in Screening require an additional confirmation.

Should a sample show a preliminary positive in the Screening phase, more hair is gathered from the primary sample for Extraction preparation. At this stage, drugs are extracted from hair at a notably low concentration compared to other techniques (such as urine or oral fluid), making hair drug screening a particularly challenging methodology.

Confirmation of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with preliminary positives undergo washing before confirmation as necessary. The full laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
