What it is, How it works

Hair captures a prolonged timeline of alcohol and other substances, encasing biomarkers within the strands of the growing hair. When taken from near the scalp, hair can reveal up to a roughly 3-month detection period for substances. It is straightforward to collect, challenging to tamper with, and easy to transport.

A sample of 1.5 inches consisting of roughly 200 strands of hair (comparable in size to a #2 pencil) from the scalp will yield 100mg, which is the ideal amount for both screening and confirmation tests. For EtG, add-ons, and/or testing exceeding 10 panels, 150mg of the sample is preferred. We suggest using a jeweler’s scale for weighing. If scalp hair is not available, a similar amount of body hair can be used. When discussing head hair, we mean scalp hair exclusively. Body hair refers to all other hair types (such as facial and axillary).

Process Overview

There are four primary steps involved in the lab processing of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a lab’s framework. This step includes checking that the sample was properly sealed and shipped, assigning it a random LAB (Laboratory Accessioning Number), and finalizing any extra data entry absent from an electronic chain of custody system.

Screening is a preliminary swift assessment for drugs of abuse. While cost-effective for ruling out drug use in most samples, a positive screen necessitates confirmation for court admissibility. Samples that are presumptively positive in Screening demand subsequent confirmation.

If a sample is deemed presumptively positive during Screening, more hair is then extracted from the original sample and processed for Extraction. During Extraction, drugs are derived from hair at much lower concentrations than other methods (such as urine or oral fluid), which makes hair drug testing the most complex method to execute.

Confirmation of any positive result from Screening is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The complete laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
