When collected near the scalp, hair can exhibit a detection timeframe of up to around 3 months for alcohol and drug usage. The collection process for hair is straightforward, challenging to tamper with, and convenient to send.

A sample measuring 1.5 inches containing about 200 hair strands (similar to the width of a #2 pencil) closest to the scalp will provide 100mg of hair, which is the optimal sample for both screening and verification. For EtG, additional tests, and tests exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale is recommended to weigh the sample. If scalp hair is absent, an equivalent quantity of body hair can be gathered. When mentioning head hair, this refers solely to scalp hair. Body hair encompasses all other hair types, including facial and axillary.

Process Overview

Four principal steps are part of the laboratory analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of entering a sample into the laboratory’s database. It involves confirming that the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry that is not provided by an electronic chain of custody system.

Screening is a preliminary quick evaluation for drugs of abuse. Though Screening is a cost-efficient approach to dismiss drug usage in most samples, a positive screen requires confirmation to be valid in court. Any samples that appear positive in Screening necessitate further confirmation.

Should a sample appear positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. During this phase, drugs are extracted from hair at a considerably lower concentration than in other methods (such as urine or oral fluid), making hair drug screening one of the most challenging techniques to conduct.

Verification of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicating presumptive positivity are washed before confirmation if necessary. The complete laboratory process, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation per ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
