What it is, How it works

Hair testing is now recognized as an effective method for identifying alcohol and drug consumption. Hair provides a comprehensive history of drug and alcohol intake by capturing biomarkers within the strands of the growing hair. When taken near the scalp, hair can indicate up to about a three-month detection period for drugs and alcohol. It is easy to obtain, not easily tampered with, and straightforward to transport.

A sample length of 1.5 inches with roughly 200 hair strands (similar to the diameter of a #2 pencil) at the scalp is needed for the best results, giving 100mg of hair for screening and confirming. For EtG, additional tests, or tests beyond 10 panels, 150mg of the specimen is recommended. Use a jeweler’s scale for accurate weighing of the sample. If scalp hair cannot be obtained, collect an equivalent amount of body hair. Note that head hair refers exclusively to scalp hair, while body hair includes all other types (e.g., facial, axillary).

Process Overview

The lab processing of a drug test result consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is brought into the laboratory's system, ensuring it was properly secured and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data needed beyond an electronic chain of custody system.

Screening is a preliminary check for drug substances. Though cost-effective in ruling out drug presence in most cases, any positive screens must be verified to be valid in legal situations. Any samples showing presumptive positive results in Screening require a secondary confirmation.

Presumptive positive samples in the Screening phase undergo further preparation for Extraction. At this stage, drugs are extracted from hair at significantly lower concentrations than other testing methods (e.g., urine or oral fluid), which makes hair drug screening one of the most complex methodologies.

For any positive screening results, Confirmation is performed using GC/MS, GC/MS/MS, or LC/MS/MS methodologies. Presumptive positive samples are washed before confirmation as required. The complete laboratory process, from Accessioning to Confirmation, receives review under the CAP (College of American Pathologists) Hair accreditation and ISO / IEC 17025 standards.

Note: Although frequently called "hair follicle tests", these tests actually examine the strand of hair, not the follicle beneath the scalp.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
