A 1.5-inch segment of roughly 200 hair strands (about as thick as a #2 pencil) nearest the scalp will produce 100mg of hair, the optimal sample for both screening and confirmation. For EtG, additional tests, and/or panels beyond 10, a 150mg sample is advised. It's suggested to weigh the hair on a jeweler’s scale. If scalp hair can't be collected, an equivalent quantity of body hair is acceptable. "Head hair" specifically refers to scalp hair. "Body hair" encompasses all other hair types (such as facial or axillary hair).

Process Overview

There are four primary stages in lab processing of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating a sample into a lab's system. This involves ensuring the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry omitted by an electronic chain of custody system.

Screening entails an initial swift check for drugs of abuse. Though Screening is a cost-efficient strategy to rule out drug usage for most samples, a positive screen must be validated for court admissibility. Any samples that test presumptively positive during Screening need further confirmation.

For samples showing a presumptive positive during Screening, additional hair is taken from the original specimen for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug testing the most challenging method to conduct.

Confirmation for any positive screen result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as needed. The complete lab process, from Accessioning to Confirmation, is assessed in line with both the CAP (College of American Pathologists) Hair standards and the accreditation to ISO / IEC 17025 requirements.
