What it is, How it works

Hair testing is now recognized for its efficacy in detecting drug and alcohol consumption. Hair stores a history of alcohol and drug usage by embedding biomarkers within the strand fibers. When collected near the scalp, hair can reveal drug and alcohol use within a roughly 3-month period. Hair is easy to gather, not easily tampered with, and straightforward to transport.

A 1.5-inch portion of around 200 hair strands (similar to the diameter of a #2 pencil) near the scalp provides 100mg of hair, the preferred amount for screening and validation. For EtG, supplement tests, and/or panels exceeding 10, a specimen of 150mg is advised. We suggest using a jeweler’s scale for measurement. If scalp hair isn't available, corresponding body hair may be used. When we mention head hair, we mean only scalp hair. Body hair encompasses all other varieties (like facial, axillary, etc.).

Overview of the Process

The primary stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning primarily deals with the initial integration of a sample into the lab’s system. It ensures the sample was appropriately sealed and mailed, assigns a random LAN (Laboratory Accessioning Number), and completes any data entry lacking from an electronic chain of custody system.

Screening entails a preliminary evaluation for substance abuse. While Screening serves as a budget-friendly means to eliminate drug use in most samples, a positive screen necessitates confirmation for legal acceptability. Samples deemed presumptively positive during Screening demand an additional confirmation.

If a sample tests positive during Screening, further hair is extracted from the initial sample for preparation in Extraction. Here, drugs are isolated from hair in much lower concentrations compared to other techniques (such as urine or oral fluid), which makes hair drug screening notably challenging.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples expected to be positive are cleansed before confirmation as needed. The entire laboratory process, from Accessioning through Confirmation, is inspected under both the CAP (College of American Pathologists) Hair criteria and the certification of ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
