If scalp hair is unavailable, a comparable quantity of body hair can be collected. References to head hair pertain strictly to scalp hair. Other hair types (facial, axillary, etc.) fall under body hair.

Process Overview

Accessioning pertains to the initial integration of a sample into a laboratory’s framework. This phase ensures the sample was securely sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and fulfills any additional data recording absent in an electronic chain of custody protocol.

Screening serves as an initial swift assessment for substances of abuse. While it is a cost-effective method to exclude drug use in most samples, a positive screen must be verified for legal acceptance. Any presumptive positive samples from Screening necessitate subsequent confirmation.

Should a sample test presumptively positive during Screening, an additional quantity of hair is extracted from the original specimen and readied for Extraction. In this phase, drugs are isolated from hair at significantly lower concentrations than in alternative techniques (e.g., urine or oral fluid), highlighting why hair drug screening presents a methodological challenge.

Confirmation of any positive screening findings is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptively positive samples undergo washing before confirmation as required. The entire laboratory workflow, from Accessioning to Confirmation, is scrutinized per the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
