If scalp hair isn't available, a comparable quantity of body hair can be utilized. When mentioning head hair, it refers solely to scalp hair, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The drug testing laboratory process encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial handling of a sample within a lab's infrastructure. This step involves ensuring proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional information entry absent from an electronic chain of custody system.

Screening entails a preliminary quick assessment for substance abuse. Although Screening offers a cost-efficient approach for excluding drug use in most cases, a positive result requires confirmation for court admissibility. Samples yielding a presumptive positive during Screening mandate further confirmation.

In instances of a presumptive positive result during Screening, additional hair is extracted from the original sample and prepared for Extraction. In this phase, drugs are isolated from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug testing the most challenging method to execute.

Confirmed positive Screening outcomes are verified via GC/MS, GC/MS/MS, or LC/MS/MS. Each presumptive positive sample is cleansed before confirmation if necessary. The comprehensive laboratory procedure, from Accessioning to Confirmation, undergoes review under the supervision of both the CAP (College of American Pathologists) Hair designation and compliance with ISO/IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
