What it is, How it works

When collected near the scalp, hair can offer approximately a 3-month period for detecting alcohol and drugs. It is easy to collect, resistant to tampering, and simple to transport.

A 1.5-inch sample comprising roughly 200 strands of hair (comparable to the size of a #2 pencil) near the scalp provides 100mg, the ideal quantity for analysis and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, a sample size of 150mg is recommended. It's advisable to measure the sample with a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair may be used. When mentioning head hair, we refer specifically to scalp hair. Body hair encompasses all other types (facial, armpit, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially integrated into the laboratory’s system. This consists of checking the sample's seal and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening conducts an initial rapid assessment for common drugs of abuse. Although Screening offers a cost-efficient method for excluding drug use in most samples, a positive screen necessitates confirmation to secure admissibility in legal settings. Any samples with presumptive positive results in Screening require further confirmation.

If a sample indicates presumptive positivity during Screening, additional hair is extracted from the initial sample and processed for Extraction. At this stage, drugs are separated from hair at significantly lower concentrations than alternative methods (e.g., urine or oral fluid), making hair drug testing the most challenging methodology to execute.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples undergo washing prior to confirmation, as required. The entire laboratory procedure, from Accessioning to Confirmation, is conducted in compliance with CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
