What it is, How it works

The identification of drug and alcohol use through hair analysis has become recognized as an effective method. Hair serves as a record of alcohol and drug consumption by embedding biomarkers within the hair strand as it grows. When samples are taken close to the scalp, hair can offer up to around a three-month period for detecting alcohol and drugs. It's easy to collect, relatively hard to tamper with, and simple to send.

A sample of 1.5 inches, containing about 200 strands (approximately the size of a #2 pencil) nearest to the scalp, will yield 100mg of hair, which is the optimal amount for testing and validation. For EtG, add-ons, and/or examinations exceeding 10 panels, collecting 150mg of the material is advised. We suggest using a jeweler’s scale to measure the sample. In cases where scalp hair isn’t obtainable, a similar quantity of body hair can be used. When we mention head hair, it specifically means scalp hair. Body hair encompasses all other types (such as facial or axillary hair).

Process Overview

The laboratory process for drug test results comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a sample into a lab’s system, ensuring the sample was properly sealed and transported, assigning it a random LAN (Laboratory Accessioning Number), and completing any required data entry not available through an electronic chain of custody.

Screening acts as a preliminary and rapid test for drug abuse presence. While cost-effective in excluding drug use in most samples, a positive outcome in Screening needs confirmation for legal admissibility. Samples showing presumptive positivity during Screening necessitate further confirmation.

If a sample shows presumptive positivity in Screening, additional hair is extracted from the original sample for Extraction. During this process, drugs are isolated from hair at concentrations lower than those in other methods (such as urine or oral fluids), making hair drug testing the most complex method to conduct.

The confirmation of any positive results from Screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that presumptively test positive undergo washing before confirmation, as required. The whole laboratory sequence from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair category and accredited according to ISO / IEC 17025 standards.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
