A 1.5-inch segment consisting of roughly 200 hair strands (approximately a #2 pencil's thickness) collected nearest to the scalp will yield 100mg of hair, which is the optimal sample for both screening and confirmation stages. For EtG, additional tests, and/or assessments exceeding 10 panels, a 150mg sample is advised. Measuring the sample's weight with a jeweler’s scale is suggested. If head hair is not accessible, a similar amount of body hair may be gathered. When discussing head hair, we mean scalp hair exclusively. Body hair denotes all other types (facial, axillary, etc.).

Process Overview

The four key steps in the laboratory handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step where a sample is entered into a lab’s system. This process involves checking that the sample was properly sealed and transported, elucidating a random LAN (Laboratory Accessioning Number), and completing any additional data recording not covered by an electronic chain of custody system.

Screening entails an initial, quick evaluation for substance misuse. While Screening is a cost-efficient method to eliminate drug use for most samples, a positive screen necessitates confirmation for it to be legally admissible. Samples showing presumptive positives in Screening must undergo further confirmation.

For samples deemed presumptively positive during Screening, additional hair is taken from the original sample and readied for Extraction. In this phase, drugs are extracted from the hair at concentrations lower than those used in other methods (e.g., urine or oral fluid), making hair drug screening the most complex method to execute.

For any positive Screening outcomes, Confirmation is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicated as presumptive positives undergo a wash before Confirmation as necessary. The complete processing from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair specification and compliance with ISO / IEC 17025 standards.
