What it is, How it works

When obtained near the scalp, hair offers a detection window of up to about 3 months for alcohol and drugs. Collection of hair is straightforward; tampering is rather difficult, and shipment is uncomplicated.

A 1.5-inch specimen consisting of around 200 hair strands (akin to a #2 pencil size) closest to the scalp yields 100mg of hair, which is the preferred amount for both screening and verification. For tests like EtG, additional testing, and/or more than 10 panels, 150mg of the substance is advised. Utilize a jeweler’s scale for weighing. Where scalp hair is not available, a comparable amount of body hair might be used. When we discuss head hair, we specifically mean scalp hair only. Other types of hair are collectively referred to as body hair (e.g., facial, axillary, etc.).

Process Overview

The lab processing of a drug test encompasses four fundamental stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed within a lab's system. This step checks that the sample remains sealed and correctly shipped, assigns a LAN (Laboratory Accessioning Number) randomly, and finalizes any additional data entry not already facilitated by an electronic chain of custody system.

Screening entails a preliminary, rapid analysis for drugs of abuse. Screening offers a budget-friendly method to eliminate drug use in the majority of samples; however, a positive result must be verified for court acceptance. Presumptive positives from Screening necessitate subsequent confirmation.

If preliminary Screening detects a presumptive positive, extra hair is extracted from the original specimen and readied for Extraction. At this stage, drugs are extracted from hair at a significantly lower concentration than in other techniques (e.g., urine or oral fluid); hence, hair drug screening is deemed the most challenging technique to execute.

Confirmation of any positive Screening outcomes occurs via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before Confirmation as required. The complete lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
