What it is, How it works

A sample measuring 1.5 inches and consisting of roughly 200 hair strands (approximating the size of a #2 pencil) closest to the scalp will yield 100mg of hair, the optimal amount for both screening and confirmation purposes. For EtG, additional tests, and/or panels exceeding 10, a specimen weight of 150mg is advised. It is recommended to measure the specimen's weight using a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair is an acceptable alternative. References to head hair exclusively mean scalp hair. Body hair encompasses all other types of hair (facial, axillary, and so on).

Process Overview

The core stages involved in examining a drug test outcome in a lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary stage for integrating a sample into a lab's system. This step includes checking the sample's seal and shipping condition, allocating a unique LAN (Laboratory Accessioning Number), and filling in any missing information not already captured by an electronic chain of custody.

Samples that are presumptively positive during Screening necessitate further confirmation.

If Screening reveals a sample to be presumptively positive, additional hair is extracted from the initial sample and prepped for Extraction. This phase involves retrieving drugs from hair at much lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening a notably complex method to execute.

Each positive screening outcome undergoes Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples may be cleansed as required before confirmation. The entire laboratory procedure, from Accessioning to Confirmation, is scrutinized under both CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
