Process Overview

There are four primary stages in processing a drug test in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails incorporating a sample into the lab’s system. This step confirms the sample's seal and proper shipment, assigns a LAN (Laboratory Accessioning Number), and finishes any additional data entry missing from an electronic chain of custody.

Screening includes a preliminary drug abuse check. Although it is economical for ruling out drug presence in most samples, a positive must be verified for legal matters. Positive screens from the first test need further confirmation.

For any sample showing presumptive positivity in Screening, more hair is retrieved for Extraction. At this stage, drugs are isolated from hair at significantly lower concentrations than in other techniques (like urine), underscoring the complexity of hair drug analysis.

Confirmation of positive screenings is achieved with GC/MS, GC/MS/MS, or LC/MS/MS. Samples preliminarily positive are cleaned before confirmation as necessary. The full process from Accessioning to Confirmation is checked under CAP (College of American Pathologists) Hair guidelines and accredited to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
