For EtG, supplementary tests, or tests exceeding 10 panels, a 150mg sample is advisable. It is suggested to measure the sample with a jeweler’s scale. If scalp hair is unavailable, an equivalent quantity of body hair can be used. Specifically, head hair pertains to hair from the scalp, while body hair encompasses all other types such as facial and axillary hair.

Process Overview

The laboratory analysis of a drug test result typically involves four significant stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the laboratory system. This process involves ensuring the sample is properly sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional information not provided by the electronic chain of custody system.

Screening conducts a fast, preliminary evaluation for drugs of abuse. While cost-efficient for discounting drug usage in most cases, a positive screening result requires confirmation to be legally acceptable. Any samples that test as potentially positive during Screening require further confirmation.

Should a test show a potential positive result in the Screening phase, additional hair is taken from the original sample for Extraction. Here, the concentration of drugs is extracted from the hair at much lower levels than in other testing methods (such as urine or oral fluid), making hair drug analysis the most challenging method to execute.

To validate a positive screening result, Confirmation is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that are potentially positive undergo washing before confirmation if necessary. The entire laboratory workflow from Accessioning through Confirmation complies with both the CAP (College of American Pathologists) Hair qualifications and ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
