Process Overview
The drug testing laboratory process consists of four key stages: Accessioning, Screening, Extraction, and Confirmation.
During Accessioning, samples are initially processed within a laboratory's system. This step involves ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.
Screening offers a preliminary evaluation for drugs of abuse. Although it is a cost-efficient method to eliminate drug use in most samples, a positive result from Screening requires Confirmation to be valid in legal contexts. Samples that test presumptively positive in Screening require additional confirmation.
When a sample tests presumptively positive at the Screening stage, more hair is extracted from the original sample and prepped for Extraction. Drugs are extracted at a much lower concentration in hair than via other methods (e.g., urine or oral fluid), highlighting the complexity of performing hair drug screening.
Confirmation of any positive result from screening uses GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed prior to confirmation if necessary. The laboratory procedure from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and meets ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.
The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
