What it is, How it works

When collected close to the scalp, hair can reveal a detection timeframe of up to about 3 months for alcohol and other substances. Hair is easy to gather, challenging to tamper with, and convenient for transportation.

A 1.5-inch sample, comprising roughly 200 strands of hair (about the diameter of a #2 pencil), nearest to the scalp will yield 100mg of hair, an optimal sample size for screening and confirmation. For EtG, additional tests, and/or evaluations beyond 10 panels, 150mg of the specimen is advised. It's advisable to weigh the specimen using a jeweler’s scale. If hair from the scalp isn't available, a comparable amount of body hair may be gathered. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Summary

The quartet of principal steps in a lab’s procedure for a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample within a lab’s framework. This step includes ascertaining that the sample was sealed and transported correctly, assigning a random Laboratory Accessioning Number (LAN), and completing any further data entry absent from an electronic chain of custody system.

Screening entails a preliminary swift inspection for drugs of abuse. Although Screening is a cost-efficient method to dismiss drug usage in the bulk of samples, a positive screen does require verification to be valid in court. Any samples perceived as positive in Screening necessitate a follow-up confirmation.

Upon a presumptive positive result in Screening, additional hair is taken from the originating specimen for Extraction. Here, drugs are withdrawn from hair at a markedly lower concentration compared to other techniques (ex. urine or oral fluid), hence why hair drug analysis is considered the most intricate technique to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive specimens are cleansed before confirmation as necessary. The entire laboratory operation from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 benchmarks.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
