What it is, How it works

When gathered near the scalp, hair can offer up to about a 3-month identification timeframe for substances and alcohol. Hair collection is straightforward, challenging to adulterate, and simple to transport.

A segment of 1.5 inches comprising roughly 200 hair strands (comparable to the width of a #2 pencil) from the scalp will yield 100mg of hair, the optimal specimen for assessment and verification. For EtG, additional tests, and/or assessments beyond 10 panels, 150mg of the sample is advised. We advocate weighing the sample on a jeweler's scale. If scalp hair is inaccessible, an equivalent portion of body hair might be acquired. Referring to head hair means scalp hair exclusively. Body hair pertains to all other hair variants (facial, axillary, etc.).

Process Outline

Samples suspected as positive in Screening need further validation.

If a sample appears positive initially in the Screening phase, additional hair is derived from the primary sample and readied for Extraction. Here, substances are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug analysis a challenging method to conduct.

Validation of any initial positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All potential positive samples undergo washing before confirmation if required. The full lab procedure from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
