A 1.5-inch strand collection of roughly 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the ideal amount for analysis and verification. For EtG, additional tests, or tests exceeding 10 panels, 150mg of hair is advisable. Using a jeweler’s scale to weigh the sample is recommended. If scalp hair is absent, an equivalent amount of body hair can be collected. When mentioning head hair, we mean scalp hair exclusively. Body hair references encompass all other hair kinds (facial, axillary, etc.).

Process Overview

The essential stages in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a sample into a lab’s system. This step verifies that the sample was properly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any further data entry not supported by an electronic chain of custody system.

Screening entails an initial quick assessment for drug abuse. Although Screening is an economical approach to dismiss drug usage in most samples, positive screens must be confirmed to be courtroom acceptable. Any specimens preliminary positive in Screening necessitate an additional confirmation.

If a sample is preliminary positive in the Screening step, more hair is extracted from the original sample and set for Extraction. Here, drugs are drawn from hair at a substantially lower concentration than other methodologies (e.g., urine or oral fluids), making hair drug screening one of the most demanding methodologies to conduct.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS or LC/MS/MS. All preliminary positive samples are cleansed before confirmation as required. The entire lab procedure from Accessioning to Confirmation is reviewed according to the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
