A sample size of 1.5 inches, consisting of roughly 200 strands close to the scalp, yields 100mg of hair, the preferred amount for screening and confirmation. For EtG, additional testing, and tests exceeding 10 panels, a sample of 150mg is advisable. It's suggested to measure the weight using a jeweler's scale. If head hair is not accessible, an equivalent volume of body hair may be used. By head hair, we specifically mean hair from the scalp. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The procedure in the laboratory for processing a drug test comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing when bringing a sample into the laboratory's workflow. This step involves ensuring the sample was properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry not included by an electronic chain of custody mechanism.

Screening involves a preliminary examination for substance misuse. While being a cost-effective means to eliminate potential drug use across most samples, any positive screen requires further confirmation to be legally valid. Any samples that test as potentially positive during Screening necessitate additional confirmation.

If a sample is initially flagged as positive during Screening, additional hair is retrieved from the specimen and prepared for Extraction. Confirmation of positive screening outcomes is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptively positive are cleansed before confirmation, as needed. The complete laboratory process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair standards and is accredited to ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
