Acquiring a 1.5-inch sample of roughly 200 strands, about the diameter of a #2 pencil, closest to the scalp yields 100mg, which is the optimal quantity for testing. For assessments such as EtG, additional tests, and/or those exceeding 10 panels, a recommendation of 150mg is suggested. It's advisable to weigh the sample using a jeweler’s scale. If scalp hair isn’t obtainable, a similar quantity of body hair may be used. When mentioning head hair, it specifically pertains to scalp hair. Body hair is a term for all other forms (such as facial, axillary, etc.).

Process Overview

The primary procedures in laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial input of a sample into a laboratory’s system, ensuring it was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entries beyond what's provided by an electronic chain of custody system.

Screening represents a preliminary rapid assessment for substances of abuse. As a cost-efficient method to exclude drug use in most samples, any positive result in Screening requires confirmation for it to be court-admissible. Samples that appear positive in Screening necessitate further validation.

When a sample shows presumptive positivity in the Screening phase, more hair is extracted from the initial sample for the Extraction phase, wherein substances are extracted at a significantly lower concentration compared to other processes (e.g., urine or oral fluid), making hair drug screening the most complex method.

Confirmation of any positive Screening outcomes is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positivity are washed prior to confirmation if necessary. The laboratory procedure from Accessioning through to Confirmation is evaluated under both CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
