What it is, How it works

When extracted near the scalp, hair can showcase a detection range of about three months for both alcohol and drugs. Hair is straightforward to gather, relatively resistant to tampering, and convenient to transport.

A 1.5-inch section containing around 200 hair strands (approximately the circumference of a #2 pencil) nearest to the scalp yields 100mg, which is the perfect sample size for analysis and verification. For EtG, additional elements, and/or tests exceeding 10 panels, 150mg of the sample is advised. We propose weighing the sample using a jeweler’s scale. If head hair isn’t accessible, a similar volume of body hair can be obtained. Head hair specifically refers to hair on the scalp. Body hair includes all other types of hair (such as facial, underarm, etc.).

Process Overview

The laboratory analysis of a drug test result includes four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a specimen into a lab's workflow. This process confirms the specimen's secure sealing and correct shipment, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional information entry absent from an electronic chain of custody system.

Screening involves an introductory assessment for illicit substances. Although Screening is a budget-friendly method to eliminate drug use in most cases, a positive result necessitates confirmation for legal validation. Any samples that yield a presumptive positive in Screening must undergo further confirmation.

If a sample is presumptive positive during Screening, additional hair is extracted from the original sample for the Extraction phase. During this phase, drugs are isolated from hair at concentrations lower than other techniques (e.g., urine or oral fluids), making hair drug testing the most complex method to conduct.

The confirmation of any positive outcome from Screening is executed using GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation as applicable. The comprehensive laboratory sequence from Accessioning through Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair categorization and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
